CSC Form 6 (Application for Leave): which box, what to attach
CS Form No. 6 is the one form a government employee uses to ask for any kind of leave. This page is for a rank-and-file employee, or an HR staff member, who needs to know which leave type to tick and what must go with it. At the end you will have the form filled and the right attachments in the envelope.

Which version
The CSC posts the CS Form No. 6, Revised 2020 on its website. If someone hands you an older edition, ask HR whether your agency still accepts it.
The form's own instructions say the application is made on this form and accomplished "at least in duplicate with documentary requirements". Everything below about days and attachments is from those printed instructions in the CSC form.

Leave of 30 calendar days or more, and terminal leave, also need a clearance from money, property and work-related accountabilities, per the form's footnote. Monetization of 50% or more of accumulated credits needs a letter to the agency head giving valid reasons. Terminal leave needs proof of resignation, retirement or separation.
Step by step
Open CSC Form 6 and go through it in order.
1. Items 1 to 5. Office or department, name, date of filing, position and salary. 2. 6.A Type of leave. Tick the leave you are taking. Use "Others" only if it is not listed. 3. 6.B Details. For vacation or special privilege leave, tick Within the Philippines or Abroad and specify. For sick leave, tick In Hospital or Out Patient and write the illness. For special leave benefits for women, specify the illness. For study leave, tick Master's degree, BAR/Board review, or other purpose. 4. 6.C Working days and inclusive dates. Count working days, not calendar days, then list the dates. 5. 6.D Commutation. Tick Not Requested or Requested, then sign. 6. Part 7. Not yours. 7.A is the certification of leave credits, 7.B the recommendation, 7.C and 7.D the decision.
The certification of leave credits (7.A)
HR fills this. It shows, as of a date, your total earned vacation and sick leave, less this application, and the balance, signed by the authorized officer. Leave it blank when you submit. A missing signature there is a common reason an application waits, so check it comes back signed.
Mistakes that get the application returned
- Using an old edition of the form. - Counting calendar days in 6.C. - Sick leave of more than 5 days, or filed in advance, with no medical certificate. - No clearance on a leave of 30 days or more. - Skipping the Philippines or abroad entry for vacation leave. - Solo parent leave without an updated Solo Parent ID. - Special emergency leave filed after the 30-day window.
Questions
Pwede bang mag-file ng sick leave bago pa ako magkasakit?
The form allows sick leave filed in advance, for a scheduled procedure for example. In that case, or when it exceeds five days, it must come with a medical certificate.
Ilang araw ang maternity leave?
The form prints 105 days, citing RA 11210 and its IRR, with proof of pregnancy attached. Ask HR about allocating credits (CS Form 6a).
What is mandatory or forced leave?
Five days of vacation leave a year. The form says unused days are forfeited, but if the head of agency cancels the schedule because of the exigency of service, it is not deducted from your accumulated vacation leave.
Do I fill out the leave credits section?
No. Part 7.A is certified by the authorized officer. You complete items 1 to 6.
