SSS Maternity Notification: how to file it with your employer

This page is for an employed member who has just confirmed a pregnancy. The Maternity Notification is the first paper step: you give it to your employer with proof of pregnancy, and your employer reports it to SSS. At the end you will know what to attach, how to fill the form, and what happens next.

This site fills the form; you give it to your employer and SSS handles the rest.

Who files it

- Employed members fill this form (SIC-01841, edition 11-2023, titled "For employed member"). Under SSS rules, she informs her employer of the pregnancy and expected delivery date with the Maternity Notification and proof of pregnancy. The employer then notifies SSS through its My.SSS account (SSS Maternity benefit). - Self-employed, voluntary, non-working spouse and OFW members do not give this to an employer. SSS says they may notify SSS directly through their My.SSS account, the SSS mobile app or a Self-Service Express Terminal (SSS Maternity benefit).

Sending the notification does not by itself guarantee payment. The form's own reminder says payment follows SSS policy (SSS Maternity Notification form).

When

Immediately after the pregnancy is confirmed (SSS Maternity benefit). To qualify for the benefit, SSS requires at least three monthly contributions in the 12 months before the semester of the childbirth, miscarriage or emergency termination of pregnancy (SSS Maternity benefit). The benefit itself is set by the Expanded Maternity Leave Law, RA 11210.

What to attach

Any one proof of pregnancy (SSS Maternity benefit):

1. A pregnancy test result signed by the physician or municipal health officer, or 2. The result of another diagnostic test: ultrasound, or a blood pregnancy test (Beta HCG or early pregnancy factor).

SSS says the employer does not have to send this proof to SSS, so it stays with your employer.

Step by step

Open the SSS Maternity Notification. The form says to fill it in one copy, write "N/A" where something does not apply, and initial any erasure (SSS Maternity Notification, instructions).

Part A, Member's data. SS number, common reference number if any, date of birth, TIN, full name, mailing address, phone, mobile number and e-mail. Use the same name as your SSS record.

Part B, Optional allocation of leave credits. Tick Yes and write the number of days only if you will give some of your leave to the child's father or an alternate caregiver. SSS allows up to seven days, deducted from your own maternity credits. The caregiver may be a relative within the fourth degree or your current partner sharing the same household (SSS Maternity benefit). The form's reminder says allocation is allowed only if that person is currently employed. SSS says the person must show their employer your written notice of the number of days, and that allocation does not apply to miscarriage or emergency termination (SSS Maternity benefit). If you will not allocate, tick No.

Part C, Certification. Write your expected date of delivery, then print your name, sign and date.

After you file

The benefit is a daily allowance equal to 100% of your average daily salary credit. SSS lists 105 days for live childbirth, 120 days for a solo parent under RA 8972, and 60 days for miscarriage or emergency termination (SSS Maternity benefit). Your employer advances the full payment within 30 days from the filing of the leave application, and SSS reimburses it (SSS Maternity benefit). The claim itself is a separate online application through My.SSS (SSS Maternity benefit).

Mistakes to avoid

- Giving the form to HR without any proof of pregnancy. - Allocating days to a father or caregiver who is not employed. - Filing the notice late and assuming the claim is protected. Ask HR the same week. - A name or SS number that differs from your SSS record.
