Moved house: update your address with SSS, PhilHealth, Pag-IBIG and BIR
You moved, and your SSS, PhilHealth, Pag-IBIG and BIR records still show the old address. Letters, pension checks and BIR notices go there. This guide takes the four updates in the order that costs least, and explains the one that can be a real errand: BIR, when your new address falls under a different Revenue District Office (RDO).
Not for you: your voter registration or driver's licence. COMELEC and LTO handle those, not these four agencies.
When to do it
Do it once the new address is where you actually receive mail. None of the four forms below gives a filing deadline for a change of home address, so we are not stating one. If you run a business, the BIR address is your registered business address, so ask your RDO before you move it.
What to bring
- The new address, complete: unit or house number, street, subdivision, barangay, city, province, ZIP code. - Your SSS number, PhilHealth number (PIN), Pag-IBIG MID number and TIN. - A valid ID. SSS's list says to show the original or a certified true copy when you hand in photocopies (SSS E-4).
The four updates

1. SSS: Form E-4
On the E-4, the new address goes in Part I-A (the member's information) and you tick Address under "Updating of contact information" (SSS E-4). The form's list of documentary requirements says updating contact information needs no required documents, apart from your ID. Fill two copies and submit to the nearest SSS branch, per the form's instructions.
2. PhilHealth: the PMRF
The PMRF is used to register and to update. Tick Updating/Amendment as the purpose, and in the updating section tick the box for personal information or address. Write the new permanent home address and mailing address in Part II. The form's instructions ask you to attach supporting documents where the update calls for them (PhilHealth PMRF). An address change alone does not involve a dependent, so none of those apply.
3. Pag-IBIG: Member's Data Form
Pag-IBIG's Member's Data Form has separate fields for your permanent address, your present address and your preferred mailing address. Fill all three. Read the form's own instruction 10 first, though: it says that for any subsequent change of information you should accomplish the Member's Change of Information Form (MCIF, HQP-PFF-049) and submit it to a Pag-IBIG branch. If a branch tells you to use the MCIF, that is the same instruction.
4. BIR: Form 1905
BIR Form 1905 is the registration update form (BIR: registration update). Go to item 7, part B, Change in registered address, and tick one of two boxes:
- Transfer within same RDO — your new address is still under your RDO. - Transfer to another RDO — write your old RDO and the new RDO.
Which RDO covers your new address is set by BIR, not by you. Ask your old RDO, or look it up on BIR's site, before you tick.
What the 1905 lists as requirements (BIR Form 1905, October 2025):

If someone files for you, the form also lists a Special Power of Attorney and both IDs.
If you are an employee, your employer usually files for you; give HR the new address and your TIN. Check the RDO before you file.
Mistakes that send the form back
- Different spelling of the address across forms. Copy the same address onto all four. - Ticking "same RDO" when it is not. Check the RDO first. - Filing the Pag-IBIG form when the branch wants the MCIF. Ask at the counter. - Forgetting the 1905 copies. A business transfer needs 3 originals for the old RDO.
Questions
Do I need to update all four agencies?
Only the ones that have you on record, but all four use your address for mail and notices, so most people do all four.
How do I know if my new address is under another RDO?
BIR assigns RDOs by address. Ask your current RDO, or check BIR's list before ticking either box on the 1905.
Is the BIR 1905 free?
The form lists fees only for a TIN card replacement, a lost COR or ATP, and closure documents. It lists none for a change of address.
Can my employer file these for me?
For BIR, yes: the form lists the documents a representative needs. For SSS, PhilHealth and Pag-IBIG, HR often forwards the forms, but you can also file at a branch yourself.
