Moved house: update your address with SSS, PhilHealth, Pag-IBIG and BIR: your forms

You moved to a new address and want your government records to follow, including your BIR registration if your city changed.

Forms in this pack: Update SSS (E-4); Update PhilHealth (PMRF); Update Pag-IBIG (Member's Data Form); Update BIR (1905).

Update SSS (E-4): Your new address and contact details.

Update PhilHealth (PMRF): Tick Updating/Amendment and give the new address.

Update Pag-IBIG (Member's Data Form): Your new address.

Update BIR (1905): Your new address; if it falls under another BIR office (RDO), this also transfers your registration there.

A change of registered address — including a transfer to another Revenue District Office — is made with BIR Form 1905.

Not for: Changing your voter registration or driver's licence (COMELEC and LTO handle those).
