Pag-IBIG membership: get your MID number and fill the Member's Data Form

Your Pag-IBIG MID number is the number every Pag-IBIG service asks for: savings, loans, MP2. This guide is for a first-time member (a new job, a business, or a stay abroad), and for a member who needs to change details later. At the end you will know which form to use, what to bring, and how to get your MID back if you lost it.

PDFPhile fills the forms. You submit them to Pag-IBIG yourself.

Do you already have a MID?

Check before you register again. Pag-IBIG's membership page says that if you forgot your MID number you can retrieve it (Pag-IBIG membership page):

- through Pag-IBIG's official Facebook page, - by emailing the Contact Center at contactus@pagibigfund.gov.ph, - or at any branch, bringing one valid ID.

The page also links an online MID inquiry on Virtual Pag-IBIG. If you worked before, an old employer may have registered you already.

Which form does what

Form · Printed title · What it is for · Source; FPF095 · Membership Registration Form (MRF) · Starting registration. The form has a membership category box (OFW or self-employed) and asks for your name, mother's maiden name, civil status, TIN, SSS/GSIS number and address · form text, drafted 02/2010; PFF039 · Member's Data Form (MDF), HQP-PFF-039 · Your full member record: category, personal details, employment, heirs · Pag-IBIG MDF; PFF049 · Member's Change of Information Form (MCIF) · Any later change to your record · MCIF

The MRF itself says registration is not complete until you also accomplish the MDF, either through the online Membership Registration System or as a submitted paper form. In other words, the MDF is the form that matters most. Pag-IBIG's membership page now sends new members to Virtual Pag-IBIG first (membership page). That page does not list the MRF, so ask your branch whether it still wants one.

Registering

Pag-IBIG's steps (membership page):

1. Register on Virtual Pag-IBIG. 2. You get a Registration Tracking Number (RTN). 3. Two to three working days later, go back to Virtual Pag-IBIG and verify to get your MID number.

If you register on paper, the MDF's own instructions say to accomplish one copy only, and to submit a photocopy of at least one valid ID acceptable to the Fund. If you registered online, the MDF should be printed back to back on a single sheet (MDF instructions).

Filling the Member's Data Form

Open the Member's Data Form. The form's own rules: type or print in capital letters, and fields marked with an asterisk (*) are mandatory (MDF instructions).

- Occupational status and membership category. Tick Employed or Unemployed/Not yet employed. If you are not employed, or the form is for pre-employment, the form says to pick Unemployed/Not yet employed. First-time jobseekers also tick the first-time jobseeker box. - Personal details. Write your name as in your birth certificate. The name extension means Jr., II, III and the like. Father and mother names are also as in the birth certificate, with your mother's maiden name. - Address and contact. Permanent and present address, mobile number, and a preferred mailing address. - Employment details. Occupation, employer name and address, and monthly income if you are employed. - Heirs. The form follows the rules of succession under the New Civil Code. Use another sheet if you need more rows. - Signature and date over the certification, which also covers your data privacy rights under RA 10173.

The form's disclaimer says registering does not automatically qualify you for loans and other programs. You still have to meet each program's requirements.

Updating your details later

The MDF's instruction 10 says to use the MCIF (HQP-PFF-049) for later changes and submit it to a Pag-IBIG branch. Pag-IBIG's page lists what can be changed this way (membership page):

- membership category, name, birth date, marital status - address or contact details, employment details, heirs - place of birth, mother's maiden name, or sex

Photocopies must be shown with the original or a certified true copy. A document issued abroad must be apostilled if the country is in the Apostille Convention, or certified by the Philippine embassy or consulate if not. Pag-IBIG lists the exact supporting document per change on that page, so check it before you go.

Two people with duplicate records can ask Pag-IBIG to merge them. Pag-IBIG lists three requirements: the Request for Consolidation/Merging of Member's Records, one valid ID, and a photocopy of your SSS employment history.

Mistakes that cause delays

- Registering twice instead of retrieving the MID. - A name that differs from the birth certificate. - Leaving a mandatory (*) field empty. - Bringing no valid ID photocopy. - Using the MDF to change details after registration, when the form says to use the MCIF. - Forgetting the date next to the signature.

Questions

Paano ko makukuha ang MID number ko?

Verify on Virtual Pag-IBIG two to three working days after you get your RTN. If you forgot it, retrieve it through the Facebook page, by email, or at any branch with one valid ID (membership page).

FPF095 o PFF039, alin ang kailangan ko?

The MDF (PFF039) is the one that completes your record. The MRF (FPF095) says on its face that you must also accomplish the MDF to finish registration. Ask your branch whether it still asks for the MRF.

Pwede ba akong magpa-register kahit wala pa akong trabaho?

The MDF has an Unemployed/Not yet employed option, with a separate box for first-time jobseekers (MDF instructions).

Paano baguhin ang apelyido ko pagkatapos ikasal?

Use the Member's Change of Information Form for a name correction, with the supporting documents Pag-IBIG lists for it. See also changing your name after marriage.

May bayad ba ang registration?

This guide found no fee stated on Pag-IBIG's membership page, so it does not give one. Ask Pag-IBIG.
