SSS E-4: how to change or correct your member data

The E-4 Member Data Change Request is the form for fixing or updating what SSS has on file: your name, birth date, civil status, contact details, bank details and beneficiaries. This page is for an SSS member whose record is wrong or out of date, for example after marriage. At the end you will know which part of the form to use, which documents to bring, and what can be left to the branch.

Check your records first

SSS compares your request with your civil registry documents. The form accepts a birth certificate or passport as the proof for a name or birth date correction (SSS E-4, list of documentary requirements). If the error is in the PSA birth certificate itself, fix it at the PSA first; the E-4 cannot correct the source. Then fix your SSS record so it matches.

What the E-4 can change

Part I-B of the form has these sections (SSS E-4):

What to bring

Always present the original or certified true copy together with the photocopy (SSS E-4). Per the form's list:

- Name or birth date correction: birth certificate or passport. If you have neither, a certificate of non-availability of birth records plus any two listed IDs or documents, both with the correct name and at least one with a birth date. - Birth certificate registered after the 55th birthday (birth date correction): two IDs or documents from that list. - Totally different name or middle name: a joint affidavit of two persons who know the facts, saying the two names are one and the same person and why the name was used. - Name change due to re-marriage: the new marriage certificate plus, as applicable, the death certificate of the previous spouse, a certificate of finality of annulment or nullity, a court order on presumptive death, or a decree of divorce. - Correction of sex: birth certificate, passport, your own received copy of the E-1, or a court order.

For a plain change of address, phone or e-mail the form lists no extra documents. The SSS Citizen's Charter asks the filer for a valid ID: one primary ID, or any two IDs with signature and at least one with photo (SSS Citizen's Charter 2026).

Online or at a branch?

SSS's pages say two slightly different things, so check which applies to you:

- SSS's Employees page says changes in member data should be reported by submitting the E-4 with supporting documents, and that "simple corrections may be done through the My.SSS Account" (SSS Employees). - The Citizen's Charter lists a service for filing the E-4 for simple corrections over the counter at branches. Its simple list includes spelling errors in a name, suffix or prefix, change from single to married, completing a middle name, gender, contact and address updates, beneficiaries, and temporary to permanent. Branch steps carry no fee, and the Charter shows no processing time for the whole request (SSS Citizen's Charter 2026). - Anything outside that list, such as a birth date correction, goes to the branch.

If an online option exists for your change, My.SSS will show it after you sign in. If not, take the E-4 to your branch.

Step by step

Open the SSS E-4. Print everything in capital letters (SSS E-4).

1. Part I-A. Your SS number, birth date and current name and address as SSS has them now. 2. Part I-B. Fill in only the sections you are changing. For a name correction write the old spelling under FROM and the new one under TO. Female members changing civil status and name fill both FROM and TO. 3. Beneficiaries (section I). Tick New/Additional or Deletion per row. The form has three rows and tells you to use another page if there are more. 4. Part I-C, Certification. Print your name, sign and date. If you cannot sign, fingerprints need two witnesses with address and contact number.

Leave Part II to SSS. Ask for your received copy; the Charter says the branch issues the duly received member's copy. Keep it.

Mistakes that get a request returned

- Wrong or missing document for the change you ticked. - Ticking several sections when you only meant one. - Typing errors on SSS's side. A member posted publicly that SSS encoded a hyphenated married surname wrongly even though the E-4 was clear. Write the surname exactly as on your marriage certificate, check the posted record in My.SSS afterward, and keep your received copy as proof. - Using an old version of the form from a third-party site. Use SSS's own copy. - Reporting a name change but not the matching birth certificate or marriage certificate.

Questions

Pwede ba online ang E-4?

SSS says simple corrections may be done through My.SSS (SSS Employees), but the Charter describes filing the E-4 at a branch. Check My.SSS first, then the branch.

Anong dokumento kapag nagpalit ako ng apelyido pagkatapos ikasal?

The E-4 section E, single to married, and section B for the name change. For name changes the form lists a birth certificate or passport as basic proof. Ask your branch whether they also want the marriage certificate.

Ilang araw bago ma-update ang record ko?

The Charter gives no single total time for the whole request, and the branch can return a request as incomplete. Ask for your received copy and check My.SSS later.

Magkano ang bayad?

The Charter lists "None" as the fee for the branch steps of this service (SSS Citizen's Charter 2026).
