Fix a wrong name or birthdate on your SSS, PhilHealth, Pag-IBIG and BIR records: your forms
Your name, birthdate or civil status is different on your PSA birth certificate and on an agency record, and a claim, loan or ID is stuck because of it.
Forms in this pack: First, if the PSA record itself is wrong: correct it at the civil registrar; Correct your SSS record (E-4); Correct your PhilHealth record (PMRF); Correct your Pag-IBIG record (Member's Data Form); Correct your BIR registration (1905).
First, if the PSA record itself is wrong: correct it at the civil registrar: Agencies follow your PSA birth certificate. A clerical error, your first name, or the day or month of birth or sex (when clearly a clerical mistake) is corrected by petition at the local civil registrar where the record is kept, without going to court.
Correct your SSS record (E-4): Name, date of birth, civil status — matched to your PSA birth certificate.
Correct your PhilHealth record (PMRF): Tick Updating/Amendment and give the details exactly as on your PSA birth certificate.
Correct your Pag-IBIG record (Member's Data Form): The same details, the same spelling.
Correct your BIR registration (1905): Your registered name and other personal details.
A clerical or typographical error, a first name or nickname, and the day and month of birth or sex (where the mistake is patently clerical) can be corrected by the city or municipal civil registrar or a consul general without a court order (RA 9048 as amended by RA 10172). Anything else needs a court order.
The petition is filed by a person with a direct and personal interest, with the local civil registry office of the city or municipality where the record is kept (RA 9048, section 3).
Not for: A change of surname, nationality or year of birth that needs a court order, or a record held for something other than your details.
