Just married: update your name and status everywhere: your forms
You got married and want SSS, PhilHealth, Pag-IBIG and BIR to show your new civil status (and your new surname, if you take it).

Get your PSA marriage certificate: Every update below asks for it. It reaches PSA weeks to months after the wedding; until then, ask the local civil registrar for a certified copy.
Update SSS (E-4): Change your civil status and surname, and add your spouse as a beneficiary.
Update PhilHealth (PMRF): Tick Updating/Amendment: civil status, surname, and your spouse as a dependent.
Update Pag-IBIG (Member's Data Form): Your new name, civil status and spouse.
Update BIR (1905): Change your registered name and civil status. Employees can ask HR to file it.
Taking your husband's surname is a choice, not a duty: a married woman MAY use her maiden name and add his surname, or use his surname (Civil Code, Article 370). If you keep your maiden name, you only update your civil status and spouse.
Not for: A change of name by court order or a correction of a typo in your PSA record (those go through the local civil registrar first).
