Register a cooperative with CDA: your forms
You are organizing a cooperative and need the papers the Cooperative Development Authority names for registration.

Cooperative name reservation request: The CDA FAQ still names a paper Cooperative Name Reservation Request Form. MC 2020-20 also has an online Cooperative Reservation form in E-CoopRIS; a reserved name expires after four days if the next step has not started.
Articles of Cooperation: MC 2020-20 says the online cooperative information form generates the Articles of Cooperation. This blank is headed Articles of Cooperation.
By-laws: Generated with the articles. The paper is headed By-laws of the cooperative.
Economic survey: MC 2020-20 names the Economic Survey among the papers that form generates. The FAQ asks for an economic survey in the Authority's format. This blank is headed Economic Survey.
Treasurer's affidavit (cooperative): MC 2020-20 names one Treasurer's Affidavit. This blank is that affidavit for a cooperative: the treasurer certifies the share capital under the cooperative's by-laws.
Treasurer's affidavit (cooperative union): The same affidavit where the treasurer certifies the capital contribution collected to register a cooperative union. The circular speaks of one affidavit; the union paper is the one that says Cooperative Union.
After an account, the applicant chooses New Registration. The next step generates the articles, the by-laws, the economic survey, and the treasurer's affidavit. After approval, the hard copies are submitted within thirty days.
The FAQ also asks for a surety bond from a registered insurer and a pre-membership seminar certificate. Neither is a CDA file in this pack.
