An employed member notifies the employer. For home confinement the employee tells the employer within five calendar days, and the employer tells SSS within five calendar days of that notice. For a hospital confinement the employer tells SSS within one year of discharge. Ordinary submission over the counter is no longer required; the employer notifies SSS through My.SSS.
A self-employed, voluntary, OFW, or separated member files in My.SSS under Benefits, Sickness Benefit. The download page still lists the paper titles. Sickness notification, sickness reimbursement, and the sickness benefit application for an unemployed, self-employed, or voluntary member are on that list, and those files were not matched to a labelled record.
Not for: Maternity. Those titles are a separate pack. Disability is a different claim and is not in the retiring pack.
